Customer REQUEST

The Slingshot Socket™ is designed for long-term durability and to take whatever life throws at it. In the unlikely event you need replacement parts, nearly all of the Slingshot Socket™ components are modular and replaceable. To request replacement parts, please submit the warranty request form below detailing the required parts for your device.

The Slingshot Socket™ parts and materials warranty covers defects in material, workmanship, and construction, which are reported to Slingshot Bionics within 90 days after the original order.

This warranty is void if the product:
1. Was not purchased from Slingshot Bionics or an authorized distributor.
2. Has been altered in any way.
3. Was not used in accordance with Slingshot Bionics’ Training or Instructions for Use.
4. Was used in an application outside of a custom medical device.

This Limited Warranty does not cover damages resulting from accidents, neglect, misuse, operation beyond capacity, improper installation, substitution of unapproved parts, or any alteration or repair by others that, in Slingshot Bionics’ judgment, significantly affects the product.

Under this Limited Warranty, Slingshot Bionics’ sole responsibility is to either repair or replace the defective product at no charge or credit the purchaser’s account for the product’s value, at Slingshot Bionics’ discretion.
This warranty does not cover any liability for personal injury, property damage, or special, incidental, or consequential damages related to the use of the product, even if Slingshot Bionics was advised of the potential for such damages.
